Hay fever is a type of allergic rhinitis caused by pollen or spores. Allergic rhinitis is a condition where an allergen (something that causes an allergic reaction) makes the inside of your nose inflamed (swollen).

  About the condition


Hayfever


Allergies are caused by an over-sensitive immune response. The immune system normally protects the body against harmful substances such as bacteria and viruses. Allergy symptoms occurs when the immune system reacts to substances (allergens) that are generally harmless and in most people do not cause an immune response.

What is Hayfever?

When a person with allergies breathes in an allergen such as pollen or dust, antibodies are produced. When the antibodies are stimulated by pollen and dust, histamine and other chemicals are released. This causes itching, swelling, and mucus production. Symptoms vary from person to person. Very sensitive individuals can experience hives or other rashes.

Hayfever involves an allergic reaction to pollen. A similar reaction occurs with allergy to mold, animal dander, dust, and similar inhaled allergens.

The pollens that cause hay fever vary from person to person and from region to region. Large, visible pollens are seldom responsible for hayfever. Tiny, hard to see pollens more often cause hayfever. Examples of plants commonly responsible for hayfever include:
           * Trees (deciduous and evergreen)
           * Grasses
           * Ragweed

The amount of pollen in the air can play a role in whether hayfever symptoms develop. Hot, dry, windy days are more likely to have increased amounts of pollen in the air than cool, damp, rainy days when most pollen is washed to the ground.

The Symptoms

           * Coughing
           * Headache
           * Itching nose, mouth, eyes, throat, skin, or any other area
           * Runny nose
           * Problems with smell
           * Sneezing
           * Stuffy nose (nasal congestion)
           * Tearing eyes
           * Sore throat
           * Wheezing

Treatment Options

The goal of treatment is to reduce allergy symptoms caused by the inflammation of affected tissues. The best "treatment" is to avoid what causes your allergic symptoms in the first place. It may be impossible to completely avoid all allergens to which you are sensitive, but you can often take steps to reduce exposure.

Medication options include the following:
           * Short-acting antihistamines can relieve mild-to-moderate symptoms but can also cause drowsiness. Many may be bought without a prescription. A pediatrician should be consulted before using these medicines in children as they may affect learning. Loratadine (Claritin) is now available over the counter. It does not tend to cause drowsiness or affect learning in children.
           * Longer-acting antihistamines are available by prescription. They cause less drowsiness and can work just as well as short-acting antihistamines. They usually do not interfere with learning. These medications include fexofenadine (Allegra), and cetirizine (Zyrtec).
           * Nasal corticosteroid sprays work very well for people with symptoms not relieved by antihistamines alone. These prescription medications include fluticasone (Flonase), mometasone (Nasonex), and triamcinolone (Nasacort).
           * Azelastine (Astelin) is a new, nasal antihistamine that is used to treat allergic rhinitis.
           * Decongestants may also be helpful in reducing symptoms such as nasal congestion, but they should not be used for long periods.
           * Cromolyn sodium is a nasal spray (Nasalcrom) for treating hayfever. Eye drop versions of cromolyn sodium and antihistamines are available for itchy, bloodshot eyes.
           * The leukotriene inhibitor Singulair is a prescription medicine approved to help control asthma and to help relieve the symptoms of seasonal allergies.

 
  About the Medicines

Various Options

There are numerous medicines that can be used to relieve the symptoms of hay fever. Many of these are now available over-the-counter from pharmacies and your pharmacist will be able to advise on which are most suitable for you.

The choice will often depend on which symptoms trouble you the most.

Antihistamine tablets and syrups
Antihistamines prevent the histamine your body produces to the allergen from causing the allergic symptoms.

They are good at relieving sneezing, itching and runny nose and eye symptoms, but are slightly less effective at reducing a blocked nose.

There are two main types:
           * those that cause drowsiness, such as chlorphenamine (eg Piriton) and promethazine (eg Phenergan)
           * newer medicines that cause less or no drowsiness, such as acrivastine (eg Benadryl allergy relief), cetirizine (eg Zirtek allergy tablets) and loratadine (eg Clarityn allergy tablets).

Many people prefer to use the newer medicines because they can usually be taken once daily and allow you to get on with your daily activities without problems.

Many antihistamines can be bought over-the-counter, but some, eg fexofenadine (Telfast), are available on prescription only.

Antihistamine nasal sprays
An antihistamine that is used directly in the nose is azelastine (Rhinolast nasal spray). It is only available on prescription.

This can be used to provide rapid relief of sneezing, itching and runny nose, but has no effect on other symptoms such as itchy eyes.

It can also be used regularly to prevent nasal symptoms, but is less effective than nasal corticosteroids.

Nasal corticosteroids
There are two nasal corticosteroid medicines that can be bought from pharmacies: beclometasone (eg Beconase hayfever nasal spray) and fluticasone (eg Flixonase allergy nasal spray). Others are also available on prescription only.

Nasal steroids reduce inflammation in the nasal passages and are better than oral antihistamines at relieving most nasal symptoms, including a blocked nose. They also relieve eye symptoms.

Nasal steroids have to be used regularly to be effective. They are best started a couple of weeks before the pollen season begins.

Nasal cromoglicate
Nasal sprays containing sodium cromoglicate (eg Rynacrom nasal spray) are also used to prevent nasal symptoms, ideally starting treatment a couple of weeks before the pollen season.

They are less effective than nasal corticosteroids, but are often the first choice for young children.

Nasal decongestants
Nose drops and sprays containing decongestants such as xylometazoline (eg Otrivine nasal spray, Sudafed decongestant nasal spray) can be bought over-the-counter to treat a blocked nose.

They should only be used for a few days, otherwise they can cause ‘rebound congestion’ when you stop using them.

Antihistamine eye drops
Antihistamine eye drops can be useful if eye symptoms are your biggest problem. They include antazoline (Otrivine antistin), which you can buy from pharmacies, and azelastine (Optilast), epinastine (Relestat), ketotifen (Zaditen) and olopatadine (Opatanol), which must be prescribed by your doctor.

The drops provide rapid relief from itchy, red, watery eyes.

Cromoglicate eye drops
Drops containing sodium cromoglicate (eg Clarityn allergy eye drops, Optrex allergy eye drops) or nedocromil sodium (Rapitil eye drops) should be used regularly, as with nasal cromoglicate products, to prevent the allergic reaction occurring.

Cromoglicate drops can be bought from pharmacies and are also suitable if eye symptoms prevail.

Use of Medicines

Steroid nasal spray
Avamys and Beconase, need to be used for a few days before they start to work. They are for continuous daily use through the hay fever season. Maximim use 12 weeks.

Antihistamine nasal spray
Rhinolast, works within hours and can be used on an as needed basis. Maximim use 12 weeks.

Rapitil and Sodium Cromoglycate are anti-inflammatory drops which work best if used regularly. Maximum use 12 weeks.

Optilast is an antihistamine. Maximum use 12 weeks.

Dosage

Here are simple tips to help you live a better life with hayfever:

1. Calm down
2. Exercise more
3. Eat well
4. Cut down on alcohol
5. Sleep well


Treatment

It is very difficult to completely avoid pollen or spores. However, reducing your exposure to the substances that trigger your hay fever should ease the severity of your symptoms. Follow the advice below to avoid being exposed to excessive amounts of pollen and spores.

           * Avoid cutting grass, playing or walking in grassy areas, and camping.
           * Wear wraparound sunglasses to stop pollen getting in your eyes when you are outdoors.
           * Change your clothes and take a shower after being outdoors to remove the pollen on your body.
           * Try to stay indoors when the pollen count is high (over 50). See Symptoms for an explanation of the pollen count.
           * Keep windows and doors shut in the house. If it gets too warm, draw the curtains to keep out the sun and keep the temperature down.
           * Do not keep fresh flowers in the house.
           * Vacuum regularly, ideally using a machine with a HEPA (high-efficiency particulate air) filter.
           * Damp dust regularly. Dusting with a wet cloth, rather than a dry one, will collect the dust and stop any pollen from being spread around.
           * Keep pets out of the house during the hay fever season. If your pet does come indoors, wash it regularly to remove any pollen from its fur.
           * Do not smoke or let other people smoke in your house. Smoking and breathing in other people's smoke will irritate the lining of your nose, eyes, throat and airways, and can make your symptoms worse.
           * Keep car windows closed. You can buy a pollen filter for the air vents in your car. This will need to be changed every time the car is serviced.
