They affect more than 15% of the UK population. Around two-thirds of migraines are in women. Migraines affect more people than diabetes, epilepsy and asthma combined.

  About the condition


Migrianes


A migraine is a throbbing, intense headache in one half of the head. It can affect people of all ages. The cause of migraine is not known.

During an attack, the blood vessels in the brain dilate and then draw together with stimulation of nerve endings near the affected blood vessels. These changes to the blood vessels are probably what cause the pain, but migraine is still a condition that is poorly understood.

Causes of Migraines

A lot of people get migraines -- about 11 out of 100. The headaches tend to first appear between the ages of 10 and 46. Occasionally, migraines may occur later in life in a person with no history of such headaches. Migraines occur more often in women than men, and may run in families. Women may have fewer migraines when they are pregnant. Most women with such headaches have fewer attacks during the last two trimesters of pregnancy.

A migraine is caused by abnormal brain activity, which is triggered by stress, certain foods, environmental factors, or something else. However, the exact chain of events remains unclear.

Scientists used to believe that migraines were due to changes in blood vessels within the brain. Today, most medical experts believe the attack actually begins in the brain itself, where it involves various nerve pathways and chemicals. The changes affect blood flow in the brain and surrounding tissues.

Migraine attacks may be triggered by:
           * Alcohol
           * Allergic reactions
           * Bright lights
           * Certain odors or perfumes
           * Changes in hormone levels (which can occur during a woman's menstrual cycle or with the use of birth control pills)
           * Changes in sleep patterns
           * Exercise
           * Loud noises
           * Missed meals
           * Physical or emotional stress
           * Smoking or exposure to smoke

Certain foods and preservatives in foods may trigger migraines in some people. Food-related triggers may include:
           * Any processed, fermented, pickled, or marinated foods
           * Baked goods
           * Chocolate
           * Dairy products
           * Foods containing monosodium glutamate (MSG)
           * Foods containing tyramine, which includes red wine, aged cheese, smoked fish, chicken livers, figs, and certain beans
           * Fruits (avocado, banana, citrus fruit)
           * Meats containing nitrates (bacon, hot dogs, salami, cured meats)
           * Nuts
           * Onions
           * Peanut butter

This list may not include all triggers.

True migraine headaches are not a result of a brain tumor or other serious medical problem. However, only an experienced health care provider can determine whether your symptoms are due to a migraine or another condition.

Diagnosis of Migraines

The first-ever attack of migraine can look like many other conditions, including meningitis or even a stroke so great care can be needed in making the initial diagnosis. Once several attacks have occurred however the pattern becomes recognisable. Depending on the circumstances it may be necessary for the person to see a specialist (neurologist) to have investigations such as a brain scan.

Other tests, such as those for ‘allergies’ or which seek to identify triggers for the migraine are of little help. Blood tests are normal in migraine.

Symptoms

Vision disturbances, or aura, are considered a "warning sign" that a migraine is coming. The aura occurs in both eyes and may involve any or all of the following:
           * A temporary blind spot
           * Blurred vision
           * Eye pain
           * Seeing stars or zigzag lines
           * Tunnel vision

Not every person with migraines has an aura. Those who do usually develop one about 10 - 15 minutes before the headache. However, it may occur just a few minutes to 24 hours beforehand. A headache may not always follow an aura.

Migraine headaches can be dull or severe. The pain may be felt behind the eye or in the back of the head and neck. For many patients, the headaches start on the same side each time. The headaches usually:
           * Feel throbbing, pounding, or pulsating
           * Are worse on one side of the head
           * Start as a dull ache and get worse within minutes to hours
           * Last 6 to 48 hours

Other symptoms that may occur with the headache include:
           * Chills
           * Increased urination
           * Fatigue
           * Loss of appetite
           * Nausea and vomiting
           * Numbness, tingling, or weakness
           * Problems concentrating, trouble finding words
           * Sensitivity to light or sound
           * Sweating

Symptoms may linger even after the migraine has gone away. Patients with migraine sometimes call this a migraine "hangover." Symptoms can include:
           * Feeling mentally dull, like your thinking is not clear or sharp
           * Increased need for sleep
           * Neck pain

 
  About the Medicine

About the treatments

People suffering a migraine should try to rest in a dark, quiet room.

Treatment of migraine is most effective if given at the start of an attack.

Simple painkillers such as paracetamol (eg Panadol), aspirin (eg Aspro clear) or NSAIDs such as ibuprofen (eg Nurofen), can be effective at relieving migraine and are usually the first treatment tried. They are preferably taken in soluble or liquid form, as these are absorbed by the body faster. If nausea or vomiting are a problem then suppositories may be preferable, alternatively an antisickness medicine can be taken with the painkiller. Migraleve contains co-codamol (paracetamol and codeine) in combination with the antisickness medicine buclizine. Alternatively your doctor may prescribe the antisickness medicines domperidone (eg Motilium) or metoclopramide (eg Maxolon); these also speed up the absorption of painkillers. There are products available that combine these with a painkiller, eg Paramax or Migramax.

If simple painkillers and antisickness medicines consistently fail to relieve migraine attacks, then the most popular modern remedies are the ‘triptan’ group of drugs. These medicines include sumatriptan (Imigran), rizatriptan (Maxalt) and zolmitriptan (Zomig), among others. They are all prescription medicines, with the exception of sumatriptan, which can now be bought from pharmacies as Imigran recovery. These drugs cause the blood vessels around the brain to contract, thus counteracting the dilatation that seems to part of the initial migraine process. They come in various forms, including nasal sprays, injections and tablets that dissolve on the tongue, which can be useful if nausea and vomiting are a problem during the migraine.

Side Effects

Common: More than 1 in 100 people who take Imigran
           * feelings of heaviness, sensations of heat or cold, pressure or tightness in any part of the body - seek medical advice if you get any of these symptoms in your chest or throat
           * sensory problems such as paraesthesia and hypoaesthesia
           * breathing difficulties
           * feeling dizzy
           * feeling drowsy
           * flushing
           * nausea
           * pain
           * raised blood pressure
           * tiredness
           * vomiting
           * weakness

Very rare: Fewer than 1 in 10,000 people who take Imigran
           * hypersensitivity reactions such as skin problems or anaphylaxis
           * abnormal laboratory test results
           * angina
           * double vision
           * ECG changes
           * eye or eyesight problems
           * faster heart rate
           * gastrointestinal problems
           * heart attack
           * heart problems
           * irregular heart rate
           * lowered blood pressure
           * palpitations
           * Raynaud's phenomenon
           * seizures
           * slower heart rate
           * stiff neck
           * stroke
           * tremors

About Sumatriptan / Imigran

Imigran contains sumatriptan succinate. It is supplied by GlaxoSmithKline UK.

Imigran 50mg tablets when used in treatment of migraine attacks

Imigran constricts blood vessels and is used to treat migraine attacks. During migraine attacks certain blood vessels in the brain become enlarged and this causes pain. Imigran constricts these blood vessels and helps to stop the pain of migraine attacks. Imigran should not be taken to prevent migraine attacks from occurring but it should be taken as soon as a migraine attack starts.

If you take a dose of Imigran for a migraine attack and it does not work you should not take another dose. If you take a dose of Imigran for a migraine attack and it works at first but then the pain comes back you can take another dose of Imigran. You must wait for at least two hours before taking this second dose. For more information about when to take Imigran and how much to take speak to your prescriber or pharmacist.

It is important that you try not to overuse Imigran. Frequent use of Imigran can make your migraines worse or make them occur more frequently.

Do not share your medicine with other people. It may not be suitable for them and may harm them.

The pharmacy label on your medicine tells you how much medicine you should take. It also tells you how often you should take your medicine. This is the dose that you and your prescriber have agreed you should take. You should not change the dose of your medicine unless you are told to do so by your prescriber.

If you feel that the medicine is making you unwell or you do not think it is working, then talk to your prescriber.

Dosage

swallow the medicine whole with a glass of water as directed by your prescriber and patient information leaflet.

If you are having problems taking this form of Imigran, you should talk to your prescriber or pharmacist. They may be able to give you advice on other ways to take your medicine or other preparations that are easier for you to take.

Warnings

Imigran is not suitable for everyone and some people should never use it. Other people should only use it with special care. It is important that the person prescribing this medicine knows your full medical history.

Your prescriber may only prescribe this medicine with special care or may not prescribe it at all if you:
           * are aged over 65 years
           * are allergic or sensitive to or have had a reaction to sulphonamides or any of the ingredients in the medicine
           * have certain types of migraine
           * have had a heart attack
           * have had or have risk factors for seizures
           * have or have risk factors for heart problems
           * have taken monoamine oxidase inhibitors in the last two weeks
           * have circulation problems
           * have galactose intolerance
           * have glucose-galactose malabsorption problems
           * have had a stroke
           * have high blood pressure
           * have kidney problems
           * have Lapp lactase deficiency
           * have liver problems
